We will soon be accepting Medicare for billing as well. At this time we will offer self pay for visits as well as a membership plans for your needs. No copays, no claims, no surprise bills. One monthly fee. This will work along side your high deductible plan and you can even use you HSA. This does not replace your insurnace plan but can compliment the high deductible plans. You do not have to have insurance: uninsured members get affordable, predictable primary care at a lower cost.

Chronic Care — $119/month Designed for individuals who need more frequent healthcare and ongoing management of chronic conditions. Includes: 6 home visits per year 12 telehealth visits per year Chronic disease monitoring Medication management Lab and imaging review Care coordination with specialists Hospital and emergency department follow-up Priority scheduling Additional home visits: $100 Additional telehealth visits: $75
Independence — $169/month Designed for older adults, individuals with mobility limitations, and patients who benefit from regular care in their own home. Includes: 12 home visits per year Up to 24 telehealth visits per year Medication reconciliation and management Chronic disease management Caregiver communication with patient authorization Care coordination Hospital and emergency department follow-up Home safety and fall-risk discussions Priority scheduling Additional home visits: $100
